As the Parent or legal Guardian of the individual attending Lekotek's Halloween Party, I fully agree to his/her participation in Lekotek’s Halloween Party and agree to release and hold harmless Lekotek, Lekotek's staff and volunteers from any and all liability, loss, damages, claims, or actions for any possible injury and/or property damage, to the extent permissible by law.

I also give permission for Lekotek staff to act in case of illness, injury and/or emergency if the parent/guardian cannot be reached.

I hereby give my permission to Lekotek to take photographs/videotapes of my child to be used for the purpose of promoting Lekotek services, training, software creation, and other uses deemed appropriate by Lekotek. Photos/Videos may be posted on Lekotek’s Facebook page, website or in printed materials.

Donate Candy, Prizes or Snacks - items needed prior to October 14th -- Please let us know if you cannot drop items off before the party. (No peanuts or peanut butter)
Bring your vehicle and participate as a Trunk or Treat on Saturday - 11 to 12:30 (No peanuts or peanut butter)
