As the Parent or legal Guardian of the individual attending Lekopals Bowling, I fully agree to his/her participation in Lekopals Bowling and agree to release and hold harmless Lekotek, Lekotek's staff and volunteers from any and all liability, loss, damages, claims, or actions for any possible injury and/or property damage, to the extent permissible by law.

I hereby give my permission to Lekotek to take photographs/videotapes of my child to be used for the purpose of promoting Lekotek services, training, software creation and other uses deemed appropriate by Lekotek. Photos/Videos may be posted on Lekotek’s Facebook page, website or in printed materials. I understand that it is my responsibility to let Lekotek Staff / Photographers know if I do not want my child to be photographed.
