As the Parent or legal Guardian of the individual attending Lekotek's SibShops, I fully agree to his/her participation in Lekotek’s SibShops and agree to release and hold harmless Lekotek, Lekotek's staff and volunteers from any and all liability, loss, damages, claims, or actions for any possible injury and/or property damage, to the extent permissible by law.

I also give permission for Lekotek staff to act in case of illness, injury and/or emergency if the parent/guardian cannot be reached.

I hereby give my permission to Lekotek to take photographs/videotapes of my child to be used for the purpose of promoting Lekotek services, training, software creation and other uses deemed appropriate by Lekotek. Photos/Videos may be posted on Lekotek’s Facebook page, website or in printed materials.
