Summary

Children will have an opportunity to visit with Santa, make some holiday crafts, play in the sensory room, and more! Volunteers are needed to help with activity stations and ensure that each child can participate and has fun!

As the Parent or legal Guardian of the individual attending Lekotek's Holiday make and take, I fully agree to his/her participation in Lekotek’s event and agree to release and hold harmless Lekotek, Lekotek's staff and volunteers from any and all liability, loss, damages, claims, or actions for any possible injury and/or property damage, to the extent permissible by law.

I also give permission for Lekotek staff to act in case of illness, injury and/or emergency if the parent/guardian cannot be reached.

I hereby give my permission to Lekotek to take photographs/videotapes of my child to be used for the purpose of promoting Lekotek services, training, software creation and other uses deemed appropriate by Lekotek. Photos/Videos may be posted on Lekotek’s Facebook page, website or in printed materials.
