As the Parent or legal Guardian of the individual attending Lekotek's Musical Saturday, I fully agree to his/her participation and agree to release and hold harmless Lekotek, Lekotek's staff, and volunteers from any and all liability, loss, damages, claims, or actions for any possible injury and/or property damage, to the extent permissible by law.

I also give permission for Lekotek staff to act in case of illness, injury and/or emergency if the parent/guardian cannot be reached.

I hereby give my permission to Lekotek to take photographs/videotapes of my child to be used for the purpose of promoting Lekotek services, training, software creation and other uses deemed appropriate by Lekotek. Photos/Videos may be posted on Lekotek’s Facebook page, website or in printed materials.
